Pet Taxi/Errand Service Agreement
This agreement is effective from _______ to ________ and is between Shadow & Marty’s Pet Care Services and

This agreement constitutes permission to enter above address and perform duties as outlined in the Taxi Information Sheet.
Any changes to this agreement must be done so in writing or they will be null and void. Shadow & Marty’s Pet Care Services has the right to make any changes to this agreement at will and without notice. With any changes, a new agreement will be presented before any new services are rendered.

Policies and Procedures
The client hereto agrees as follows:
1.
Shadow & Marty’s Pet Care Services and its employees agree to provide services stated in this contract in a reliable and trustworthy manner. In consideration of these services and as an express condition thereof, the client expressly waives any and all claims against Shadow & Marty’s Pet Care Services or its employees, unless arising from negligence on the part of Shadow & Marty’s Pet Care Services.
2.
Shadow & Marty’s Pet Care Services or its employees shall not be held responsible for the loss, injury, death, or actions of any pet while not in the care of Shadow & Marty’s Pet Care Services
3.
The client understands that all pets (where appropriate) must have a veterinarian and must be up to date on the rabies vaccination.
4.
Shadow & Marty’s Pet Care Services does not accept aggressive animals.
5.
All animals will be transported in a crate provided by Shadow & Marty’s Pet Care Services or client.
6.
All pets must be accompanied by a leash and/or crate.
7.
Client is responsible for making all arrangements with vet, groomer, boarding facility, etc. Shadow & Marty’s Pet CareServices is not responsible for actions of destination facility.
8.
Shadow & Marty’s Pet Care Services will not be responsible for any keys the client has asked to be mailed.
9.
Shadow & Marty’s Pet Care Services reserves the right to deny service or terminate service because of safety concerns, financial concerns, or inappropriate or uncomfortable situations.
10.
Shadow & Marty’s Pet Care Services reserves the right to cancel and reschedule service due to weather concerns.
11.
Business and visiting hours fall between the hours of 8 a.m. and 6 p.m. and services are usually completed during this time unless we are behind schedule. We do our best to keep to your appointment time; however, due to the nature of the service, there may be times when we are behind schedule because of an extended previous visit. We will notify the facility if this should occur. If we are unable to agree on a new appointment time with destination facility, we will reschedule for another day.
12.
Payment is expected before services are rendered. Shadow & Marty’s Pet Care Services is not responsible for any fees due to the vet, groomer, kennel, etc. Client must make prior payment arrangements. If client would like Shadow & Marty’s Pet Care Services to submit payment for client in the form of cash, Shadow & Marty’s Pet Care Services is not responsible for any loss.
13.
Cancellations must be received 2 days prior to scheduled service or a $30 cancellation fee will apply.
